A socket graft note has to show that the graft went into a fresh extraction site, at the extraction, for a reason. This D7953 template documents the extraction, the socket walls, the graft material and any membrane, closure, and the implant or prosthetic plan the graft is preserving the ridge for.

Extraction #[8] with socket graft for ridge preservation (D7953). Medical history reviewed; [no changes]. Indication for extraction: [non-restorable; vertical root fracture]. Purpose of the graft: [preserve ridge width and height for a planned implant at #8]. Pre-operative radiograph [PA #8 / CBCT]: [buccal plate present; no periapical pathology]. Risks discussed, including [pain, swelling, bleeding, infection, loss or exposure of graft particles, and ridge loss despite grafting]; alternatives reviewed, including [extraction without a graft and immediate implant placement]; [written] consent obtained. Topical anesthetic applied. Local anesthesia: [1.5] carpules of [2% Lidocaine 1:100k epi] via [buccal and palatal infiltration]. Tooth extracted [atraumatically with periotomes and forceps, without a flap]. Socket walls: [buccal plate intact] / [buccal dehiscence of [4] mm]. Socket debrided of granulation tissue and irrigated with sterile saline. [Mineralized allograft] placed and condensed into the socket. [Resorbable collagen membrane placed over the graft (D7956)] / [Collagen plug placed over the graft]. Closed with [4-0 chromic gut, criss-cross suture]; [primary closure not attempted; membrane left exposed to heal by secondary intention]. Hemostasis achieved. Post-operative instructions given verbally and in writing, including [no vigorous rinsing or spitting for 24 hours and avoiding pressure on the site]. Analgesics: [ibuprofen 600 mg with or without acetaminophen as directed]. Next visit: [post-op check in 7 to 14 days]; [re-evaluate for implant placement after graft healing].

Ambient listening records the appointment in the background. AI writes the note from the conversation itself.

“I'm Dr. Yasmin, a dentist who got tired of staying late writing notes. I used to search the internet for note templates that actually worked, and I could never find good ones. So I built ButterNote to make note writing feel as smooth as butter.
If the click-build flow above just saved you time, you'll love what's behind a free account: templates for practically every clinical scenario you'll see this week, with instant custom note generation. And if you want zero clicks at all, the iOS app handles that too. Ambient listening on your iPhone or Apple Watch turns each appointment into a finished note.

Use this socket graft note template when you place a bone replacement graft in a socket at the time of the extraction to limit ridge resorption. CDT D7953 is bone replacement graft for ridge preservation, per site. Its descriptor says the graft is placed in an extraction or implant removal site at the time of the extraction or removal to preserve ridge integrity, for example when it is clinically indicated in preparation for implant reconstruction or where alveolar contour is critical to a planned prosthetic reconstruction. It also says the code does not include obtaining graft material, and that a membrane, if used, is reported separately. D7953 is reported in addition to the extraction itself (D7140, D7210, and so on), per site.
Membranes are where claims most often go wrong. D4266 and D4267 are now described as guided tissue regeneration on natural teeth, so a barrier placed over a grafted extraction socket falls under the edentulous-area codes instead: D7956 (resorbable barrier, per site) and D7957 (non-resorbable barrier, per site). Barriers around implants have their own per-implant codes, D6106 and D6107. A collagen plug or sponge used to seal the socket is not necessarily a barrier membrane, so name exactly what you placed and check with the payer before reporting it as D7956. Coverage of D7953 itself varies widely between plans, and the planned implant or prosthesis is the reason a reviewer will look for first.
The note should describe the extraction technique (ideally atraumatic), the socket walls you found, especially the buccal plate, debridement of granulation tissue, the graft by type (allograft, xenograft, alloplast, autograft, or PRF) and product if your office tracks it, the membrane or plug, closure, and when the site will be re-evaluated for the implant. Set expectations accurately in the consent: a 2019 systematic review found that ridge preservation by socket grafting attenuates the horizontal and vertical ridge loss that follows extraction compared with extraction alone. It reduces that loss; it does not prevent it, and sites with a thin buccal wall did less well.
Record the reason for the extraction, the reason for the graft (the implant or prosthetic plan), the radiograph, risks, alternatives, and consent.
Record the anesthetic and how the tooth was removed, with or without a flap.
Record the socket walls, especially the buccal plate, and debride granulation tissue and irrigate.
Record the graft material by type and product, placed and condensed into the socket.
Record the membrane or plug, the sutures, whether primary closure was achieved, and hemostasis.
Give written post-operative instructions, book the post-op check, and record when the site will be re-evaluated for the implant.
Click-build any socket graft note in seconds, or let ambient listening turn your appointments into finished notes.

An implant placement note is the record every later visit leans on: which implant went in, how big, how tight, and how it was left to heal.
A surgical extraction note has to show why the tooth could not simply be elevated and delivered with forceps.
This tooth extraction note template covers every required step (anesthesia, technique, socket care, hemostasis) so you never miss a detail in a rushed appointment.
A sinus lift note has to show why the bone was not enough, how you got under the sinus floor, and what state you left the Schneiderian membrane in.
Second-stage surgery is short, and the note often is too.
This implant crown note template covers the restorative visit after the implant has integrated: the site, implant system and platform, abutment, screw torque, retention method, contacts and occlusion, and the baseline radiograph and probing that every future peri-implant comparison depends on.
