How do you write a dental note?
Write it in the order the visit happened: the chief complaint in the patient's words, the medical history review, your findings by tooth and surface, the diagnosis, consent, anesthesia (agent, amount and technique), what you did and with which materials, any complications, post-op instructions, the next visit, and your signature. Each template on this page follows that order for one procedure, with the details you would change in [brackets].
What are some examples of dental clinical notes?
Every template on this page is a worked example: a comprehensive exam, a composite, a crown prep and delivery, an extraction, a root canal, SRP, a prophy, perio maintenance, a pulpotomy, nitrous, a night guard delivery and more. How do I add a template to Open Dental, Dentrix, or Eaglesoft?
Copy the template from this page, then save it in your software. In Open Dental: Setup, Auto Notes, Add. In Dentrix: Template Setup on the Clinical Notes toolbar in the Patient Chart, then New Template. In Eaglesoft: Lists, AutoNotes, New. Paste the text, give it a name, and save. All three can turn the bracketed fields into fill-in prompts.
Are these dental note templates really free?
Yes. The copy-paste templates on this page are completely free, with no account required. A free ButterNote account adds click-to-build note generation and syncing, but the template text itself is always free to use in your charts.
Can I customize the templates for my practice?
Yes. Each template uses [bracketed fields] you replace with your specifics (tooth numbers, materials, anesthesia, findings), and you can freely reword any sentence to match your own documentation style before pasting it into the chart.
Do these templates work with Open Dental, Dentrix, or Eaglesoft?
Yes. The templates are plain text, so they paste into the clinical-note field of any dental software, including Open Dental, Dentrix, and Eaglesoft, as well as any other EHR or word processor. You copy the finished note and paste it wherever you chart.
Are the templates HIPAA-compliant to use?
The templates themselves contain no patient information. They are blank clinical-note scaffolds. You fill in patient specifics inside your own HIPAA-compliant chart system, and the copy-paste templates on this page never transmit that data to ButterNote.
