When to use this template

Use this crown prep clinical note template after preparing any tooth for a full-coverage indirect restoration — single-unit crown, onlay, or 3/4 crown. Works for zirconia, PFM, e.max, gold, and full-porcelain crowns. The template includes the indication (large restoration, fractured tooth, post-RCT, etc.), bite-registration step, retraction cord pack, impression material, and temporary fabrication and cementation.

Adjust the bracketed fields for your specific tooth, material, shade, and impression workflow (digital scan vs. PVS vs. alginate). For posterior crowns where shade matching is less critical you can omit the shade field, but most labs prefer a shade noted regardless. Document the indication explicitly — large restoration, fractured tooth, post-RCT, cracked tooth, or cosmetic — because this drives the medical-necessity component of insurance review.

Retraction cord size matters: the chart entry "size 0 cord packed" supports the contour-recording step and helps the lab interpret the gingival margin. For digital scan workflows, the impression material defaults to "Digital Scan" (you can specify the scanner brand in the additional-notes field). The temporary cement (TempBond NE, TempBond Original, or Reliance) is referenced again on the cementation-visit's note when the temp is removed, so noting it now keeps continuity of care.

Step by step: how to document this crown prep

    Local anesthesia delivered (type, volume, technique) and adequate anesthesia confirmed before prep.

    Bite registration and pre-op shade taken before tooth structure removal.

    Tooth refined with high-speed diamond burs to manufacturer-specified clearance for the chosen material.

    Tissue management

    Retraction cord packed (size noted) or laser troughing performed for clear margin definition.

    Take impression

    Digital scan, PVS, or alginate impression taken depending on lab workflow.

    Fabricate and cement temp

    Temporary fabricated with Protemp, Integrity, Luxatemp, or custom; cemented with TempBond.
