Free Bridge Preparation Note Template (D6740/D6245)

A bridge prep note covers more than two crown preps. This template documents why a fixed bridge was chosen for the space, how the abutments were judged fit to carry it, each abutment and pontic by tooth number, the material and shade, the impression, and the provisional bridge that protects the preparations.

When to use this template

Use this bridge prep note template at the preparation visit for a conventional fixed partial denture. In CDT each unit is its own line: the abutment crowns are retainer crowns and the replacement teeth are pontics, and the material picks the code. An all-ceramic bridge, including zirconia, is commonly reported as D6740 (retainer crown, porcelain/ceramic) for each abutment and D6245 (pontic, porcelain/ceramic) for each pontic. A porcelain-fused-to-high-noble bridge uses D6750 and D6240, and a full cast high noble bridge D6790 and D6210. Many payers treat the seat date as the date of service, so the prep note often supports a claim filed at delivery, and some plans have a missing tooth clause that excludes replacing a tooth lost before coverage began.

Start the note with the indication and the alternatives. Record which tooth is missing and since when, and that implant, removable partial denture, and no replacement were discussed, along with why the patient chose a bridge. Then document the abutment evaluation, because the bridge is only as good as the teeth holding it: periodontal support, probing depths and mobility, caries and existing restorations, pulpal or endodontic status, crown-to-root ratio, and alignment for a common path of insertion. A periapical radiograph of each abutment belongs here.

At the preparation, record the anesthesia, the reduction for the chosen material, the margin design and location, retraction, and the impression or scan of both abutments, the edentulous ridge, and the opposing arch, with a bite record. Name the shade and the pontic design, since the pontic form decides how the patient will clean under it. Finish with the provisional bridge (material and temporary cement), the occlusion check, and instructions for cleaning under the provisional pontic.

Step by step: how to document this bridge preparation

  1. 1

    Record the indication

Note the missing tooth, the alternatives discussed, and why a fixed bridge was chosen.

  2. 2

    Evaluate the abutments

Record periodontal support, mobility, caries, pulpal status, and crown-to-root ratio, with radiographs.

  3. 3

    Prepare

Anesthetize, prepare the abutments for the chosen material with a common path of insertion, and record margin design and location.

  4. 4

    Capture

Retract, then scan or impress the preparations, ridge, and opposing arch; take a bite registration.

  5. 5

    Prescribe

Record the material, shade, and pontic design sent to the lab.

  6. 6

    Provisionalize

Fabricate and cement the provisional bridge, check occlusion, and teach cleaning under the pontic.
