Free Crown Lengthening Note Template (D4249)

Crown lengthening is a surgical procedure, so the note has to read like one. This D4249 template documents the restorative reason, the healthy periodontal starting point, the flap, how much bone was removed and where, the distance left between the planned margin and the bone, closure, and the healing period before the final impression.

Clinical crown lengthening, hard tissue, #[14] (D4249). Indication: [subgingival caries and fractured palatal cusp extending close to the bone crest; insufficient sound tooth structure for a crown margin and ferrule]. Periodontal status: [healthy; probing depths 2-3mm; no bleeding on probing]. Radiograph [PA #14]: [caries about 1 mm from the alveolar crest palatally; no periapical pathology; furcation not involved]. Restorability confirmed; alternatives reviewed, including [extraction with implant and orthodontic extrusion]; risks discussed, including [root sensitivity, longer-looking teeth, open embrasures, and a change in the gum line]; consent obtained. Local anesthesia: [2] carpules of [4% Septocaine 1:100k epi] via [buccal infiltration and greater palatine block]. [Sulcular] incisions #[13-15]; full-thickness mucoperiosteal flap reflected buccally and palatally. Osteotomy and osteoplasty with [a round carbide bur under sterile saline irrigation and hand chisels]; bone removed [circumferentially] to establish [3] mm from the planned restorative margin to the alveolar crest, measured with a periodontal probe; exposed root surfaces planed. Flap [apically positioned] and closed with [4-0 Vicryl, interrupted]. Hemostasis achieved. Post-operative instructions given verbally and in writing; [chlorhexidine 0.12% rinse twice daily]; analgesics: [ibuprofen with or without acetaminophen as directed]. Next visit: [suture removal and post-op check in 7-10 days; final crown preparation after healing, about 6 weeks].

When to use this template

Use this crown lengthening note template for surgical crown lengthening that removes bone so a tooth can be restored. CDT D4249, clinical crown lengthening (hard tissue), is employed to allow a restorative procedure on a tooth with little or no tooth structure exposed to the oral cavity. It requires reflection of a full-thickness flap and removal of bone, altering the crown-to-root ratio, and it is performed in a healthy periodontal environment, as opposed to osseous surgery, which is performed in the presence of periodontal disease. A soft tissue procedure alone to gain access for a restoration is D4212 (gingivectomy or gingivoplasty to allow access for restorative procedure, per tooth), and many payers consider minor soft tissue removal part of the crown preparation.

The clinical reason is the supracrestal tissue attachment, the band of junctional epithelium and connective tissue above the bone crest that the 2017 World Workshop classification uses in place of the older term "biologic width". Restorative margins placed into it are associated with inflammation and attachment loss. So the note should state why the tooth cannot be restored as it is (subgingival caries, fracture, short clinical crown, no room for a ferrule), the periodontal status that makes D4249 the right code, the radiograph, and the alternatives discussed, such as orthodontic extrusion or extraction.

At surgery, record the anesthesia, incision design, the full-thickness flap, the osteotomy and osteoplasty with the instruments used, and the distance you established from the planned restorative margin (or sound tooth structure) to the bone crest, measured with a probe. Many clinicians aim for about 3 mm, but record the number you measured. Record root planing of the newly exposed root, flap positioning and sutures, and the post-operative care. Finally, record the healing period before the final impression: payers commonly expect around six weeks, and longer is often advised where the gingival margin position matters esthetically.

Step by step: how to document this crown lengthening

  1. 1

    Justify

Record the restorative reason, healthy periodontal status, radiograph, restorability, and alternatives.

  2. 2

    Consent and anesthetize

Record the risks discussed, consent, and the anesthetic, volume, and technique.

  3. 3

    Reflect the flap

Record the incision design and the full-thickness flap.

  4. 4

    Remove bone

Record the osteotomy and osteoplasty, the instruments, and the measured margin-to-crest distance.

  5. 5

    Close

Plane the exposed root, position the flap, and record the sutures and hemostasis.

  6. 6

    Plan healing

Record post-operative care, suture removal, and the healing period before the final impression.
