Free Incision and Drainage (I&D) Note Template (D7510)

An I&D note is an emergency note with a surgical procedure in the middle of it. This D7510 template documents the swelling and its source tooth, the systemic and airway signs you checked, where you incised and what drained, any drain or culture, your antibiotic decision, and the definitive treatment that still has to follow.

Yasmin Byott

“I'm Dr. Yasmin, a dentist who got tired of staying late writing notes. I used to search the internet for note templates that actually worked, and I could never find good ones. So I built ButterNote to make note writing feel as smooth as butter.

If the click-build flow above just saved you time, you'll love what's behind a free account: templates for practically every clinical scenario you'll see this week, with instant custom note generation. And if you want zero clicks at all, the iOS app handles that too. Ambient listening on your iPhone or Apple Watch turns each appointment into a finished note.

When to use this template

Use this incision and drainage note template when you drain an intraoral soft tissue abscess through an incision in the mucosa. CDT D7510 is incision and drainage of abscess, intraoral soft tissue, and its descriptor specifies an incision through mucosa, including abscesses of periodontal origin. D7511 is the complicated version, where dissection is extended into adjacent fascial spaces, and D7520 and D7521 are the extraoral equivalents. Drainage obtained through an endodontic access or an extraction socket is not an incision through mucosa, so it is generally documented as part of that procedure rather than as D7510.

The assessment is what makes the note safe. Record the chief complaint and duration, the location and size of the swelling, whether it is fluctuant and localized or diffuse, the source tooth with test results and a radiograph, and the systemic picture: temperature, malaise, and lymph nodes. Record the airway and spread checks as well, including the negatives: mouth opening, swallowing, breathing and voice, the floor of the mouth, and whether swelling extends toward the eye or the neck. If those signs are present, or a deep space infection or sepsis is suspected, the ADA guidance is urgent referral, and the note should show where the patient was sent.

For antibiotics, the 2019 ADA guideline recommends against them for a localized acute apical abscess in an immunocompetent adult without systemic involvement when definitive treatment, including incision and drainage, is provided. With systemic involvement such as fever or malaise, it recommends definitive treatment plus antibiotics: amoxicillin 500 mg three times daily or penicillin V potassium 500 mg four times daily, for 3 to 7 days, with follow-up after 3 days and discontinuation 24 hours after the systemic signs fully resolve. Write down which situation applied and why. Close with the definitive treatment of the source tooth, because an I&D relieves the abscess but does not treat its cause.

Step by step: how to document this incision and drainage

  1. 1

    Assess

Record the complaint, swelling, systemic signs, and airway and spread checks, including negatives.

  2. 2

    Find the source

Test the suspected tooth and its neighbors and take a radiograph; record the diagnosis.

  3. 3

    Anesthetize

Record the anesthetic and technique, avoiding injection into the abscess.

  4. 4

    Incise and drain

Incise at the most dependent point, dissect bluntly, express the pus, and irrigate; record the drainage.

  5. 5

    Drain, culture, and medicate

Record any drain or culture and the antibiotic decision with its reason.

  6. 6

    Plan definitive care

Record the definitive treatment of the source, the re-evaluation visit, and emergency instructions.
